What Happens in the Brain During Brainspotting?

Most people who come to Brainspotting therapy in Indianapolis understand intuitively that something significant is happening in their sessions. They feel it. Things move. Something that has been stuck for years begins to shift. But they are not always sure what is actually happening in the brain that produces those shifts.

This post is for the people who want to understand the neuroscience. Not because understanding it is required for Brainspotting to work, but because for many people, understanding why something works makes them more able to trust the process and stay with it.

Starting With the Basics: Two Parts of the Brain

To understand what Brainspotting does in the brain, it helps to start with a basic distinction between two regions that are central to the process.

The first is the prefrontal cortex. This is the thinking, reasoning, planning, language-using part of the brain. When you talk about your problems in therapy, analyze your patterns, or work through a cognitive reframe, you are primarily working with the prefrontal cortex. This region is powerful and its involvement in healing is genuinely valuable.

The second is the subcortical brain. This is the deeper, older part of the brain that handles survival responses, emotional processing, and the storage of experiences that are encoded before language and conscious thought catch up. The amygdala, the hippocampus, the basal ganglia, and the brainstem all live here. This region does not operate in words or logic. It operates in sensation, emotion, and automatic response.

Trauma, chronic stress, and deeply rooted emotional patterns tend to be encoded primarily in the subcortical brain. They live below the level where talking and reasoning can fully reach them. This is why people can have profound insight into their patterns and still find those patterns running automatically in their lives.

Brainspotting is designed specifically to access the subcortical brain.

The Role of the Visual Field

The mechanism that makes Brainspotting work begins with the eyes.

The visual system has an unusually direct relationship with the deep brain. The optic nerve connects to multiple subcortical structures, including the superior colliculus, which plays a role in directing visual attention, and pathways that connect to the amygdala and other limbic structures involved in emotional processing and threat response.

This means that where you look affects how you feel. Not metaphorically but neurologically. Different positions in the visual field activate different parts of the brain. And when you look at a specific position that connects to a stored experience or emotion, the brain region holding that material becomes more active and more accessible.

This is the foundation of Brainspotting. When your therapist slowly moves the pointer across your visual field and asks you to notice what changes in your body, they are essentially mapping your visual field to find the eye position that corresponds to where a specific experience is stored in your subcortical brain.

That position is your brainspot.

What Happens When the Brainspot Is Found

Once the brainspot is located and you hold your gaze there, something specific begins to happen in the brain.

The subcortical region associated with the stored experience becomes activated in a way that is different from ordinary activation. Because you are in a safe relational environment, with bilateral sound supporting the process and a therapist holding dual attunement, the activation happens without the full fight-flight-freeze response that would normally accompany accessing that material.

In neuroscience terms, this is sometimes described as activating the material while keeping the window of tolerance open. The stored experience is being touched by the brain, but not in a way that overwhelms the system.

What happens next is where the processing actually occurs. The brain begins to do what it naturally does when it is allowed to process an experience fully: it integrates it. The raw, undigested quality of the stored material begins to soften. Neural pathways that were firing in fixed, automatic patterns begin to become more flexible. New connections become possible.

Dr. David Grand, who developed Brainspotting, describes this as the brain processing and releasing what it has been holding. The analogy he often uses is that the brainspot gives the brain a location to process from, and the fixed gaze creates the sustained attention that allows the processing to go deep enough to be complete.

The Role of Bilateral Sound

Most Brainspotting sessions use what is called BioLateral sound: music that pans slowly from ear to ear, alternating between left and right.

The bilateral quality of this sound is significant. Bilateral stimulation, alternating input between the left and right hemispheres, appears to support information processing in the brain in ways that are still being studied but consistently observed clinically. EMDR uses bilateral eye movements for similar reasons. Brainspotting uses fixed gaze combined with bilateral sound, creating a dual input that appears to deepen and support the processing.

Many clients describe the BioLateral music as having a settling quality during sessions, as though it provides a kind of neurological scaffolding that makes the deeper processing feel more contained and manageable.

Neuroplasticity and Why Brainspotting Can Produce Lasting Change

One of the most important concepts in understanding how Brainspotting produces lasting change is neuroplasticity, the brain's capacity to reorganize itself by forming new neural connections throughout life.

When Brainspotting processing occurs, it appears to facilitate a kind of neural reorganization. The fixed patterns that were maintaining a trauma response, an anxiety state, or a depressive baseline begin to loosen. New pathways become established. The brain learns something different about how to process the stored material.

This is why the changes that happen in Brainspotting tend to feel different from insight. Insight is knowing something new. Neuroplastic change is the brain operating differently. Clients often describe it as things feeling different before they can explain why. The shift is in how the nervous system is running, not just in what the thinking mind understands.

For clients working with trauma, anxiety, or depression, this kind of neuroplastic change is what produces results that last beyond the therapy room and generalize into daily life.

Why Understanding This Matters for Your Healing

Knowing what is happening in the brain during Brainspotting can change how you relate to the process.

When you understand that the processing is happening at a subcortical level, it becomes easier to trust experiences in session that do not make immediate logical sense. A memory that surfaces unexpectedly. An emotion that does not seem directly connected to what you came in to work on. A physical sensation that rises and falls without a clear narrative attached to it.

All of these are the brain doing its work. And understanding that the brain knows how to process when given the right conditions tends to make it easier to stay with the process rather than trying to direct or analyze it.

The job in a Brainspotting session is simply to notice what is happening and stay present with it. The brain takes care of the rest.
